When the queue is growing faster than the completion rate, it means one of two things: engineers are writing code faster than reviewers can keep up, or reviews are stalling and creating a bottleneck that will delay the entire sprint.

A growing PR queue has a compounding effect. Stale PRs create merge conflicts. Merge conflicts require rework. Rework consumes capacity that was allocated to other stories. By day five, a PR queue problem that was visible on day two has become a sprint-wide velocity problem. Track the open-to-merged ratio daily. If the queue has grown by more than thirty percent relative to its day-one baseline by midpoint, treat it as a sprint risk signal and investigate immediately. Signal 2: Story Points In Progress Exceed Story Points Done by Day 3

Work in progress is the enemy of throughput. A common pattern in struggling sprints is that the team starts many stories quickly, gets each partially done, and then finds that nothing is actually complete by midpoint. This is visible in a simple metric: story points currently in progress versus story points marked done.

By day three of a ten-day sprint, a healthy team should have at least fifteen to twenty percent of their committed points in the done column. If you have sixty points in progress and zero points done, you have a WIP problem that will almost certainly result in missed sprint goals. The fix is not to work faster. When the engineer who owns a critical path story is blocked, the clock runs. A forty-eight-hour block on a critical path story in a ten-day sprint is the equivalent of a five-day block in a three-week sprint. The signal to watch for is the same engineer appearing in standup with the same blocker two days in a row. At that point, the dependency has become a sprint risk, and the engineering manager or tech lead needs to own the escalation rather than waiting for the blocker to resolve itself. Signal 6: Meeting Load Spiking Mid-Sprint

Engineering capacity is not just headcount. It is focused time. A senior engineer who is in five hours of meetings on a given day has roughly two to three hours of deep work available. In most sprint capacity calculations, that engineer is counted as a full day. The delta between planned capacity and actual focused time is a hidden sprint risk factor that almost no team tracks.

Meeting load tends to spike mid-sprint for a predictable reason: planning for the next sprint begins. Grooming sessions, stakeholder reviews, and cross-team syncs cluster in the middle of the current sprint, which is precisely when the current sprint needs the most focused execution capacity. Tracking calendar load against available sprint hours is not something most engineering tools do natively, but it is worth building even a rough version of this signal. A team that has absorbed three hours of unplanned meetings per engineer per day has lost twenty-five percent of its sprint capacity, and that loss will show up in the sprint results whether it is tracked or not. WIP versus done: Green if done points are greater than fifteen percent of commitment. Yellow if done points are five to fifteen percent. Red if done is below five percent.

Unplanned work: Green if unplanned work is below ten percent of capacity. Yellow if ten to twenty percent. Red if above twenty percent.

External blockers: Green if no stories have been blocked for more than twenty-four hours. Yellow if one critical path story is blocked. Red if two or more critical path stories are blocked.

Story stagnation: Green if all stories have had activity in the past twenty-four hours. Yellow if one story has had no activity in forty-eight hours. Red if two or more stories are stagnant.

Meeting load: Green if per-engineer meeting hours are within ten percent of sprint plan. Yellow if ten to twenty-five percent over plan. Red if more than twenty-five percent over plan.

A sprint with two or more red signals at day three has a high probability of missing its goals. That assessment, made on day three, gives you seven days to respond. You can descope, you can remove blockers, you can protect engineering time. The same assessment made on day nine gives you one day, and your options collapse to damage control.

How to Measure If It's Working

Improving sprint predictability requires a baseline and a consistent measurement approach. Start by tracking sprint predictability as a simple ratio: story points committed divided by story points delivered, averaged over a rolling eight-sprint window. Most teams measuring this for the first time discover their actual predictability is between fifty and seventy percent, well below what they would have estimated.

Set a target of eighty percent predictability over a rolling quarter. That is an achievable number for most teams with moderate process investment, and it is high enough to meaningfully change how stakeholders trust sprint commitments. Teams that reach eighty percent predictability consistently find that product planning conversations become easier, roadmap confidence increases, and the last-minute scramble that characterizes most sprint endings begins to disappear.

Track the scorecard results alongside the predictability metric. Over time, you should be able to correlate specific red signals with sprint outcomes. If PR queue problems reliably predict a missed sprint, that tells you where to invest in tooling or process.
